Appointment Scheduling
Our front office works very hard to accommodate each patient and their physician's requests. Please be patient, and know we will try and meet those requests. 

We confirm appointments one business day prior, it is important that our records have current contact numbers to reach you.


Telephone Messages
Our telephone lines are open throughout the day. A triage nurse is available between 8 a.m. and 5 p.m. to answer your urgent medical questions. Calls will be triaged and answered in order of medical importance. Patient calls are returned as quickly as possible. Please make sure your contact numbers are up to date in your record to ensure a prompt response to your questions and concerns. When leaving a message, please indicate if our office can leave a message with your medical information. For your privacy, if you have not indicated verbally or in writing, our office will not leave a message with medical information at your contact number. Non-urgent messages or test results that require a practitioner to call you back are typically returned by that practitioner after patient care hours are completed. They too are answered according to medical priority. 

IF AT ANY TIME YOU FEEL THAT YOUR QUESTION MAY BE A MEDICAL EMERGENCY, CALL 911 OR GO DIRECTLY TO THE NEAREST EMERGENCY ROOM.

Insurance/Financial Information/Referrals
Currently, the healthcare industry is in significant flux and both patients and providers frequently change their participation status in insurance plans. We recommend that you remain informed as to your covered services, and that you check directly with our staff at the time that you schedule your appointment as to our participation status. Please be sure to have a current and valid insurance card available when you schedule you appointment, and when you check in for your appointment. 

Fifth Avenue Gynecologists currently participates in most insurance plans, including Medicare and several AHCCCS plans. If our office is considered "out of network" in your plan, this does not preclude you from seeing us. Being seen by an out-of-network provider often obligates you to different financial responsibilities under your plan. Please familiarize yourself with these responsibilities. If you need help with these clarifications please do not hesitate to contact our billing staff.

Our staff is sensitive to the rising cost of healthcare and understands that financial concerns prevent some patients from seeking professional medical care. We have alternative payment options, if necessary, to fit most budgets. However, payment for services is expected at the time of your appointment. Our business office is able to assist you Monday through Thursday 8am – 5pm, and Friday from 8am – 12 pm at (602) 896-4307.

If you have any questions regarding your health insurance benefits, be sure to contact your insurance company at the Customer Service number located on your insurance card.

If your insurance plan requires a referral to be seen in our office, you are responsible for obtaining the authorization and bringing it to your appointment. Contact your insurance company or primary care physician before scheduling an appointment.
